Community Health Needs Assessment (CHNA)

The Community Health Needs Assessment (CHNA) defines priorities for health improvement, creates a collaborative community environment to engage stakeholders, and an open and transparent process to listen and truly understand the health needs of the community served by CHI Memorial in Tennessee and North Georgia.


Funding Requests 

CHI Memorial is currently accepting applications from non-profit agencies for consideration for fiscal year 2024 (July 1, 2023 – June 30, 2024). The committee will meet throughout the year to determine eligibility and availability of funds. We will contact applicants with updates throughout the year. CHI Memorial’s guidelines for community support, funding application and funding evaluation are available for download. 

Improving Health Care Access

CHI Memorial partners with Project Access, the Chattanooga-Hamilton County Health Department, the Homeless Health Care Clinic, the Samaritan Center, Catholic Charities and other agencies to ensure that medical care is available for everyone. CHI Memorial Community Health - Hixson offers primary and preventive care and social services regardless of ability to pay.

Electronic Request for Records

CHI Memorial now offers an online patient record request tool that verifies your identity by asking for a photo of your driver’s license, which can be taken via webcam or smartphone. There is no additional charge to use this service. Please note: Chrome, Safari and Firefox are the recommended browsers for this application.

To request a copy of your medical information in person or by fax, download, print and complete the “Authorization for Use or Disclosure of/Access to Protected Health Information” form (to authorize disclosure of records to another person or organization) or “Patient Access Request to Their Protected Health Information” form (to request access to own records).  You may bring the form to the CHI Memorial Medical Records Department (on the 1st floor of CHI Memorial Hospital Chattanooga), fax it to (423) 495-4740 or mail it to:

The No Surprises Act protects consumers who get coverage through their employer (including a federal, state, or local government), through the Health Insurance Marketplace® or directly through an individual health plan, beginning January 2022, these rules will:

  • Ban surprise billing for emergency services. Emergency services, even if they’re provided out-of-network, must be covered at an in-network rate without requiring prior authorization.
  • Ban balance billing and out-of-network cost-sharing (like out-of-network co-insurance or copayments) for emergency and certain non-emergency services. In these situations, the consumer’s cost for the service cannot be higher than if these services were provided by an in-network provider, and any coinsurance or deductible must be based on in-network provider rates.
  • Ban out-of-network charges and balance billing for ancillary care (like an anesthesiologist or assistant surgeon) by out-of-network providers at an in-network facility.
  • Ban certain other out-of-network charges and balance billing without advance notice. Health care providers and facilities must provide consumers with a plain-language consumer notice explaining that patient consent is required to get care on an out-of-network basis before that provider can bill the consumer.


For consumers who don’t have insurance, these rules make sure they’ll know how much their health care will cost before they get it, and might help them if they get a bill that’s larger than expected.

The rules don’t apply to people with coverage through programs like Medicare, Medicaid, Indian Health Services, Veterans Affairs Health Care, or TRICARE because these programs have other protections against high medical bills.

Cost Estimator

Our Standard Charges file includes information from our hospital chargemaster and payer contracts but does not reflect a patient’s financial liability. If you have insurance, we encourage you to contact your insurance provider to confirm your payment responsibilities, deductibles, and other details of your insurance plan. You may also be eligible for financial assistance.

The Standard Charges file is intended to comply with the CMS price transparency requirements. A brief description of the key pricing terms as defined by are outlined below:

  • “Gross Charge” is the charge for an individual item or service that is reflected on the hospital's chargemaster, absent any discounts.
  • “Discounted Cash Price” is the charge that applies to an individual who pays cash (or cash equivalent) for a hospital item or service.
  • “Payer-specific negotiated charge” is the reimbursement rate that the hospital has negotiated with a third-party payer for an item or service.
  • “De-Identified Maximum Negotiated Charge” is the highest reimbursement rate that a hospital has negotiated with all third-party payers for an item or service.
  • “De-Identified Minimum Negotiated Charge” is the lowest reimbursement rate that a hospital has negotiated with all third-party payers for an item or service.

  • The hospital charges listed do not include patient-specific insurance deductibles or copayments, potential self-pay, or financial assistance discounts and therefore are not a reflection of patient responsibility.
  • The pricing is in effect as of the applicable period prior to the publication date; pricing is subject to change. Any changes to charges, rates, network participation, or other data elements on or after the publication date are not reflected. There may be differences between the Charge Description Master and Standard Charges files due to update timelines.
  • Actual reimbursement for services is dependent on many factors, such as the length of stay, the specific medical services provided, the medications and supplies provided with the service, and other variable factors that vary by payer and pricing methodology. Payer-specific pricing in this file does not incorporate all variable factors.
  • Payer-specific pricing does not include all alternate rates for claims which may have additional or unexpected services.

The following Standard Charges Machine Readable File is posted in a CMS-compliant .JSON file format. Per 45 CFR 180.20, Machine Readable File format means “a digital representation of data or information in a file that can be imported or read into a computer system for further processing. Registered Nurse Residency

CHI Memorial Registered Nurse (RN) Residency Program is accredited with distinction as a Practice Transition Program by the American Nurses Credentialing Center’s Commission on Accreditation in Practice Transition Programs. 

 

Accredited since 2017, CHI Memorial’s RN Residency Program was the first in Tennessee to achieve accreditation. As of February 2024, more than 870 new graduate nurses have attended CHI Memorial's RN Residency Program since our first cohort in July 2015 when our program was revamped. New nurses gain exposure to different areas, find a department that’s a great fit for both the nurse and their team and feel comfortable in their new roles as they strive to grow in their careers. 

 

Our comprehensive, 12-month program provides recent graduates of nursing schools with a consistent and thorough orientation experience designed to transition them into professional practice successfully.

 

During the orientation phase of the program, residents:

 

  • gain access to key systems
  • complete orientation checklists
  • participate in simulations 
  • complete patient care with an experienced preceptor
  • further develop critical thinking skills and increase their confidence in providing safe patient care 

 

New nurses begin their training in February, July or September each year. Education and training are unique to the clinical track. 

 

Our RN residency program is improving the quality of care provided to patients while also increasing physician confidence in new graduate nurses. 

 

 

CHI Memorial offers:

  • $30/hr base pay for new graduate nurses plus
  • $10,000 sign-on bonus for med-surg/telemetry
  • $5,000 sign-on bonus for critical care, emergency department or surgery
  • $5,000 relocation package for more than 50 miles
  • $4/hr night shift differential
  • $2/hr weekend shift differential
  • Potential to receive a $500 bonus for CHI Memorial Professional Ladder participation

 

Registered Nurse Fellowship

CHI Memorial Registered Nurse (RN) Fellowship is accredited as a Practice Transition Program by the American Nurses Credentialing Center’s Commission (ANCC) on Accreditation in Practice Transition Programs.

 

CHI Memorial began an RN fellowship program in critical care in June 2020 with the intent to provide structured training for experienced nurses who want to work in critical care departments but do not have prior experience in an intensive care environment. In fall 2023, the program expanded to include the emergency department. 

 

The RN Fellowship Program has two phases. Phase one is 12 weeks and lays the groundwork through instruction on critical care or emergency department concepts, provides precepted critical care or emergency department experience and interdepartmental shadows. Phase two follows the fellow through the remainder of his/her first year of experience in critical care or emergency department providing support and continuing education. The fellowship program has a proven track record of success for new critical care nurses at CHI Memorial.

 

The RN Fellowship Program is offered at CHI Memorial Hospital Chattanooga, Hospital Hixson and Hospital Georgia Any registered nurse who applies for a job in a critical care unit or emergency department but does not have prior critical care or emergency department experience will automatically join the fellowship program upon hire.

 

ANCC Practice Transition Accreditation validates hospital residency or fellowship programs that transition registered nurses (RNs) and advanced practice registered nurses (APRNs) into new practice settings and that meet rigorous, evidence-based standards for quality and excellence. Nurses in accredited transition programs, like CHI Memorial’s RN Fellowship Program, experience curricula that promote the acquisition of knowledge, skills, and professional behaviors necessary to deliver safe, high-quality care. 

 

Candidates are eligible for CHI Memorial’s sign-on program: 

  • $5k for full time RN position with less than one year of RN experience
  • $15k for full time night shift with more than one year of RN experience
