Advance directives, also known as an Advance Care Plan or a Living Will, can help patients communicate their treatment choices if they are otherwise unable to make such decisions.
An Advance Care Plan is a document that allows patients to tell their doctors and loved ones how they want to be treated if they are terminally ill or permanently unconscious.
