CHI Memorial Center for Healthy Aging specializes in geriatric medicine providing quality health care that helps you remain healthy and active. Medical care and medication management can become more complex as we age. In addition to providing primary healthcare for those age 55 and older, CHI Memorial Center for Healthy Aging focuses on managing and treating complex age-related conditions including memory loss, fall and balance issues and dementia-related behavioral problems. 

CHI Memorial Center for Healthy Aging has two locations. Dr. Cleinman and T. Shae Hinton, FNP, see patients at our main location in the CHI Memorial Missionary Ridge Medical Tower, 725 Glenwood Drive, Suite E-786, Chattanooga, TN 37404. Caregiver Support


If you’re a caregiver for an aging family member, CHI Memorial Center for Healthy Aging can provide support to help you navigate questions and concerns about memory loss and functional ability and provide emotional support along the way. Our comprehensive program eliminates or simplifies stressful decisions, leaving more time to spend with family, friends and community.

We recognize that being a primary caregiver can be physically and emotionally difficult. That’s why our team includes a dedicated social worker to offer support to the whole family and provide access to community resources at the appropriate time. We offer an environment where family members can talk about their concerns without feeling judged, get information they need about disease progression, and gain understanding about what is and what is not part of the normal aging process. Geriatric medicine is a subspecialty of internal or family medicine which focuses on "geriatric syndromes" - which include dementia, falls/gait instability, incontinence, polypharmacy, delirium.

We offer a consultative clinic visit for memory loss and falls assessment for patients 50 years and older; in addition to our full geriatrics assessment clinic for patients 55 years and older. We also see patients for primary care who are 55 years and older. We have a dedicated social worker who provides counseling to caregivers and assists our geriatrician with patient placement/equipment needs for our geriatric clinic patients.

Did you know changes that happen naturally as you age can impact the way medications work inside your body – and affect your quality of life? A person’s body composition naturally changes as they age; there’s more fat than muscle mass. And medications that are quickly absorbed in fat begin to last much longer in the body than they did at age 40 or 45. There’s also an increased permeability in the blood/brain barrier, meaning these medications can have much greater psychological effect than they once did. Even over-the-counter medications like Benadryl can cause confusion and memory loss.

Patient Portal


We care about our patients. Our goal is to provide you and your family with the best experience possible when interacting with our practice. We understand that it is not always possible to visit the clinic, leave message, wait on the phone, or update your personal health information. By offering online access to your health information we look forward to simplifying your healthcare experience.

My OneCare/Follow My Health is a secure, online tool that connects you directly to your personal medical record. My OneCare will enable you to:

  • Update personal information
  • Request or change appointments
  • Ask clinical questions
  • View medication and allergy lists
  • Request a prescription refill or change a pharmacy
  • View lab results, tests, clinic visits

Patients who provide an email address during a clinic visit will receive a My OneCare invitation by email. The invitation will be sent from noreply@followmyhealth.com and will include instructions on how to register. If you do not receive an invitation check your spam folder. The invitation expires after 60 days.  

Creating a OneCare / Follow My Health Account: 

  1. Click the registration link in the email you receive from noreply@followmyhealth.com 
  2. Create your portal login using a unique username and password.
  3. Enter the invitation code provided by your medical provider and click "Agree" to the release of information.
  4. If you already have a portal account, you can add providers to your existing account.  Please call (888) 670-9775 if you have any questions.

The patient portal invitation you receive after visiting one of our facilities will never expire. Once you sign up, the portal will be available to you 24/7.

You can also use another application to access your health information, if you prefer. To do so, you will need to first set up a portal account with My OneCare. The username and password you choose will be the security component for your health information and make it possible for you to access and import it.

The FollowMyHealth app is available in the Apple Store and on Google Play.
