CHI Memorial Chattanooga Internal Medicine Group is a primary care physician practice with six board-certified internal medicine physicians. Our providers emphasize disease prevention by focusing on treating the whole person and working with you to create an ideal picture of your health. Our goal is to provide comfortable, convenient and prompt care to help you get well and stay well. 

Sarah Baker, MD, Holley Davis, MD, Johannes DuPlooy, MD, Hannah Edwards, MD, Rachel Labovitz, MD, and R. Forrest Sowell, MD, treat patients age 18 and older. All of our physicians welcome new patients.

Online Check In


Did you know that our practice allows you to check in online ahead of time? You can confirm your appointment via text message or email, and check in online prior to your visit. You can also check in on your own device when you arrive. Ask us about using mobile registration to make your next visit simpler and faster!

We are partnering with Phreesia to help you save time registering for your appointment. Use mobile check-in to register from any smartphone, tablet or computer from the privacy of your own home. Checking in on your device saves you time and ensures your information is always up to date!

Frequently Asked Questions

Open communication with your health care provider ensures you receive the very best care. Follow these  tips during your next appointment:

  • Bring a list of questions or concerns to every appointment.
  • Share changes to your health, including hospitalization, and visits to a specialist or the  emergency room.
  • Bring a list of your medications, vitamins or herbal supplements.
  • If you have questions or don’t understand something discussed in your appointment,  always ask your health care provider to explain it in another way.

In order for us to care for our patients as efficiently as possible, please follow these guidelines when requesting medication refills:

  • Please let us know at the time of your visit what prescription you need refilled, and we will do that either to your local pharmacy or your mail order company.
  • As for prescription refills at the time of your appointments - for some medications that you request, a 90-day refill may not be considered appropriate by your doctor and may not be approved.
  • If you have a medication that needs a prior approval we will assist you by completing a form from your insurance company. However, we may require you to call your insurance company to obtain your prior approval form. You can fax this form to us at (423) 495-2698
  • If you need to change a medication due to your insurance coverage or the cost of co-pays, an office visit will be necessary. Please bring a list of covered medications from your insurance company. If you do not have a list, call you insurance company to obtain one before your visit.

Routine prescriptions will not be filled on weekends.

Patient Portal


We care about our patients. Our goal is to provide you and your family with the best experience possible when interacting with our practice. We understand that it is not always possible to visit the clinic, leave message, wait on the phone, or update your personal health information. By offering online access to your health information we look forward to simplifying your healthcare experience.

My OneCare/Follow My Health is a secure, online tool that connects you directly to your personal medical record. My OneCare will enable you to:

  • Update personal information
  • Request or change appointments
  • Ask clinical questions
  • View medication and allergy lists
  • Request a prescription refill or change a pharmacy
  • View lab results, tests, clinic visits

During your clinic visit you will be asked to provide an email address. You will be emailed a My OneCare invitation which includes instructions on how to register. Communications will come from the following: noreply@followmyhealth.com. If you do not receive an invitation check your spam folder. The invitation expires after 60 days.  

  1. Click the registration link in the email you receive from noreply@followmyhealth.com 
  2. Create your portal login using a unique username and password.
  3. Enter the invitation code provided by your medical organization and click "Agree" to the release of information.

If you already have a portal account, you can add our office to your existing account.  Please call (888) 670-9775 if you have any questions.

The patient portal invitation you receive after visiting one of our facilities will never expire. Once you sign up, the portal will be available to you 24/7.

You can also use another application to access your health information, if you prefer. To do so, you will need to first set up a portal account with My OneCare. The username and password you choose will be the security component for your health information and make it possible for you to access and import it.

The FollowMyHealth app is available in the Apple Store and on Google Play.
